Looks like a custom piece to me. Something from an original sculpt or an early MovieFX design?

One of the fist MFX helmets.
Made out of 3 parts.
Faceplate, Back and Cap are two single parts.
Also you can see that Guidon uses tape at this time for the teath and not painted them.
At this time he used mixer jet's from a faucet as mictips.
